How do you put it on your sidebar?

A screw-on is a small hold that is screwed onto either a feature, or the wall. A feature is a "hold" that is too big usually to be classified as a hold, on which there are often screw ons or other holds.

Some of my confusion is whether a term refers to something on the wall/rock vs something the climber does...

like whether a screw on is a type of hold, or if it is a way that a climber would grab a hold?

Yes -- assuming that a hold is the thing on the wall as opposed to what you do to it.
