Assumming you are covered by a Medicare C Plan. It should be possible to switch in the current open enrollment period to regular Medicare and then buy a Medigap plan and a Medicare D plan, both starting Jan 1, I think. Would give you choice of any doctor, no copays. More expensive premiums, but maybe something to consider?

Out-of-network seems my downfall. In-network Boston is reknowned for expertise but I chose Cleveland Clinic due to a personal intro as well as their excellent online presentation re my specific issue.

Welcome to the Midwest! I've never had a bad experience at Cleveland Clinic: great doctors, cutting edge treatments if wanted, and a caring, 'no-nonsense' staff.

And you can take the train from the Cleveland airport directly to the Clinic...with a couple of blocks of walking through bombed-out neighborhoods. And on the return trip to the airport, can get off at the Great Lakes brewery for a couple of Commodore Perry IPAs and an enjoyable lunch! Afterward, a walk through the West Side Market, a real 'blast from the past' that so few cities have retained.

Yeah but a limo has to sit in backed-up traffic. But the Red Line train from the Airport runs every 10 minutes, and will get you there in 20. And for an orienteer, a good excuse for some map reading, and navigating in an unfamiliar area.
