I've talked to a couple of people who've run it, including an older fellow in my orienteering club who originates from South Africa, and they all make it sound impressive. Isn't it run uphill one year and downhill the next? And entries are capped at 20 000 people...

Yes, I've been told the downhill is the harder option as it kills your quads. Viv said she did her one (and only) Comrades on a uphill year. Impressive number of spectators too, by the sounds of it.

Also has a strict cut off time. I saw a video a few years ago and it was agony watching someone struggle to run up to the gates of the football ground where the finish was as the clock counted down and he missed out by 5 seconds. They closed the gate, I think it was 11 hours cut off.
