| Tom_O: | My knee is not good Dasha. It still tightens up whenever I try to resume any sort of normal running schedule. I plan on going to the doctor soon.
They moved my permanent office because DuPont is terminating it's lease in the Brandywine Building and they needed to relocate everyone. I still keep my office downtown eventhough I won't use it until the project I'm working on at the contractor office in Newark wraps up sometime next year. Sounds like things are going a little more smoothly for you recently - that's good. |

| triple-double: | That's bad news about the knee. I don't remember, when did you damage this knee last time? Is it clicking or popping, or locking up? Is it swollen? Or do you think it is hamstring, or other muscle pull, and that it is more of a muscle issue than the knee itself? I hope it improves soon, or that the doctor will be helpful in getting it better.
Things are going a little bit smoother for me, really almost day by day. I really hope that in 1 month I will be doing a lot better--more efficient, will know the system, and will be more comfortable managing patients and all the little things that come up. I have 3 more days with my current team. 1 more call night there on Tuesday. And then I will be switching to a different team on Thursday. And it will be officially my first month of internship too. |

| Tom_O: | My knee problem continues to mystify me. I injured it back in April on a routine trail training run. I didn't wrench it, never heard a pop, etc. There was a mild sensation of warmth around the medial outside of my knee but it was not painful. I continued to run on it the next few weeks and it got much worse - very stiff along the inside back of the knee but no significant swelling. I thought that it was the tendon connecting the upper and lower leg and nothing inside my knee but now I'm not sure.
At one point I had convinced myself I could hear a popping noise when I moved my lower leg in a particular direction but I believe that popping is actually in my ankle and not in my knee. I still have some residual warm sensation at the back of my knee which I presume indicates some remaining inflammation. So I assume there are various possible explanations including a medial meniscus tear, arthritis, bursitis, tendonitis, etc. I just hope whatever it is will not prevent me from running long term. That would be a major lifestyle change which I'm not quite ready for! |

| triple-double: | I was thinking along the same lines of the possible explanations, but certainly none of those should prevent you from running long-term. (I always think of all those basketball players that continue to perform at such a high level after catastrophic injuries). But I really hope that it is not like Speedy's problem, either.
I agree that this could well be a tendon inflammation or bursitis (there are hamstring, and several other muscle attachments right below the knee joint on the medial side--e.g. gracilis, sartorius), and there are a couple of bursas that could be inflamed also. If it is wamer than the other side, it is definitely a sign of ongoing inflammation, and there may be some subtle swelling as well. Usually with these the first thing to try is ibuprophen and icing, and PT that does not exacerbate the pain. It does not sound like a meniscus problem. Popping and locking is usually classic for a meniscal tear, and you could try testing it by lying on your back and rotating your leg (by holding the ankle) with the knee bent to try to catch a torn meniscus edge and to reproduce the clicking/popping, etc. I hope it feels better soon! |

| Tom_O: | Thanks Dr. Dasha! I will try more stretching, icing, and vitamin I. In the meantime, I can still do things like cycling which does not aggravate it and actually seems to help. Just very frustrating. |
