Discussion: Does anyone have a meet expense/revenue form that their club uses?

We were looking at using a standardized meet financial statement/form for use by our meet directors which would include the usual expenses for prizes, printing, travel, registration fees etc.

Does anyone have or use one of these that they would share? I suspect that anyone who uses Excel or another spreadsheet regularly just makes their own. (I am not a regular Excel user)
